PRIVACY ACT STATEMENT
AUTHORITY:
5 U.S.C. § 552a (Privacy Act of 1974); 10 U.S.C. § 5013 (Secretary of the Navy); SECNAVINST 5211.5 (Department of the Navy Privacy Program).
PRINCIPAL PURPOSE:
To collect information necessary to assess human readiness, identify risk factors, and enable leadership awareness and intervention to support Sailor well-being and unit readiness.
ROUTINE USES:
Information may be used by authorized personnel within the Department of the Navy, including members of the Sailor's Chain of Command (e.g., LPO, LCPO, DIVO) and any Trusted Agents designated by the Sailor, for the purpose of maintaining readiness, providing support, and taking appropriate leadership or administrative action. Information may also be used in aggregate, non-identifiable form to assess trends and improve command-level awareness.
DISCLOSURE:
Providing information is voluntary. However, failure to provide requested information may limit the system's ability to assess risk, provide support, or enable leadership awareness of potential issues affecting readiness.
PRIVACY AND ACCESS:
Access to submitted information is restricted to authorized personnel with a legitimate need-to-know in the performance of their official duties. Unauthorized disclosure or misuse of this information may result in disciplinary action and/or criminal penalties under applicable federal law.
